progress note as requested by your doctor.

If you have new questions or a new health concerns, please call the front desk to schedule an appointment.  Fees will be incurred for care rendered through portal. 

Please be aware that portal messaging can take 48-72 hours for a response.  Portal messages are returned in the order of emergent need.  Please do not send multiple messages as it increases administrative load at our small office staff.
